Screening could save your life from colon cancer



When I was working at a hospital as an admitting officer. I encountered patients checking in with different diseases. My heart went to all the cancer patients I made friends with. Some of them died as of this writing and some of them seek advice abroad.



I learned from doctor friends that if you have symptoms like these:

1.  If there is a change in your bowel habits, including diarrhea or constipation or a change in the consistency of your stool, that lasts longer than four weeks.

2.  Rectal bleeding or blood in your stool

3.  Persistent abdominal discomfort, such as cramps, gas or pain

4.  Weakness or fatigue

5.  Unexplained weight loss


 And if you are 50 y/o and above.  Have yourself  screened by your gastroenterologist for these are the signs of having a colon cancer.

Recently, I was invited to a Colon Cancer Awareness Presscon on Feb 22 at  South Ballroom, Manila Diamond Hotel. The presscon was attended by Dr. Frederick Dy,  Dr. Judith Gapasin Tonco, Dr Dennis Ngo. 


The Philippines ranks the fastest increase in mortality rate for colon cancer incidences in the world, in both male and female.

This was revealed by gastroenterology experts from the Philippine Society of Gastroenterology, at a media conference at the Manila Diamond Hotel held to raise awareness about Filipino about colon cancer, urging them to get screened for the said disease. Westmont Pharmaceuticals, a division of Unilab, partners with PSG, in building consciousness for colon cancer, its symptoms, diagnosis and treatments available for Filipinos.


The above finding is one of the results of the study entitled Global Colon Cancer: Incidence and Mortality undertaken last year and recently published in Gut, an international gastroenterology journal.

“Colorectal cancer has certainly become a real concern in the Philippines.  The growing number of incidence and mortality worldwide have put the disease as the third most common type of cancer for both men and women men, after lung and breast cancers,’’ said Dr. Frederick Dy, gastrointestinal oncologist and a leading expert in endoscopy.
In the Philippines, colon cancer is the fifth most common type of cancer for both sexes, after breast, lung, liver, and cervix. It is the fourth leading cause of death overall.  According to the most recent statistics of World Health Organization’s (WHO) foremost cancer research and statistics project, GLOBOCAN, there were 4,901 deaths of the 8,553 incidences of colorectal cancer in Philippines in 2012, which means that more than half of those diagnosed have died from the disease.  Worldwide, colorectal cancer accounts for about 8% of cancer-related deaths, with 774,000 deaths occurring in 2015, according to WHO. The estimated number of patients that will be affected by colon cancer is 2.2 million with 1.1 million deaths annually by 2030.


Interestingly, GLOBOCAN also said that most incidences of colorectal cancer occur in highly-developed countries like in North America, Western Europe and Japan. But as more Filipinos suit up for a more western lifestyle and diet, they have become more susceptible to the disease now more than ever.

Unfortunately, the level of awareness about colorectal cancer is much lower than that for other types such as breast or lungs.  Doctors also find difficulty in urging patients to adopt lifestyle changes for the prevention of colorectal cancer.

 “Colorectal cancer is influenced by both modifiable and non-modifiable risks, one of which is lifestyle and diet.  But as a lifestyle illness, it is also asymptomatic (displaying no symptoms) in the initial stage,’’ Dr. Dy says.

Modifiable risk factors, he explains, are those that can be changed like obesity, cigarette smoking, alcoholic beverage intake, physical inactivity and increased consumption of red meat.  A diet rich in animal fat generally increases risk, while eating food rich in fiber and calcium may possible reduce the risk.

“An active lifestyle must be put in place as well as the stopping of vices in order to reduce the risk of acquiring this disease,” Dy adds.

WORSE THAN IT LOOKS
With the increase in incidences, colon cancer is now among the deadliest, and recent data show that it’s not slowing down either.

 “The data we are looking at right now seem to indicate that there are more people developing colorectal cancer.  It’s not only here in the Philippines, but also in the Asia-Pacific region—we’re seeing a rise in the number,” Dy explained.

Dy also stated the non-modifiable risks in colorectal cancer—factors that are inherent in a person like family history of the disease, and gender.  Colorectal cancer is more common in men than in women, and studies are still being undertaken to correlate colorectal cancer with other types of cancer.
“Examples of non-modifiable risk factors include gender, age, and family history.  These are factors that cannot be altered.  Some diseases like breast cancer patients or diabetes mellitus have also been associated to increase the probability of developing colorectal tumors,” said Dy.

PREVENTION IS BEST
Colon cancer, like breast cancer, is highly preventable for as long as the patient involves himself as much as possible in its prevention by ensuring a healthy lifestyle, and regularly going for medical check-ups, even before turning 50.

According to Dy, the recommended age for screening for colorectal cancer is 50—this is the age when the appearance of polyps increases.

“Colon cancer doesn’t begin as cancer immediately; it starts with pre-malignant precursors called polyps.  If left attended, these polyps grow over time, eventually becoming cancer. For young people, the risk is low, but they are not immune to it.  We have young adults developing colorectal cancer.  The occurrence is generally uncommon, but if someone has symptoms referable to colorectal cancer, no matter the age, they need to see their doctor or better if they consult a gastroenterologist. Most of colorectal cancer occurs sporadically but some have a strong genetic background.  There are also genetic syndromes that also develop a significant number of polyps at a younger age,” said Dy.

Dy explained that the when the polyps have become a full-blown cancer, the common treatments are surgery, chemotherapy, and radiotherapy, modalities which are also used to treat other forms of cancer.  But preventing colorectal cancer is the best strategy and best achieved through colonoscopy where a tube with a camera is inserted into the digestion tract to look for abnormal tissue or tumors. These small growths can be removed without the need for surgery. This may be the reason why in some countries, at least in the United States where screening is advocated, the incidence of colorectal cancer is starting to drop.

RELUCTANCE
The challenge now is to get people to take their gastrointestinal health seriously and to create a campaign which would inform and educate people about colorectal cancer as not another type of cancer.

Filipinos, especially the younger generations, are less concerned about their diet and their lifestyle, and this is reflected on a small number of Filipinos actually dedicating time going for yearly medical check-ups and follow-ups.

Another factor is the fear of going through colonoscopy, as it becomes very private to the patient.
“I talk to my patients, and I tell them that these [colonoscopy] procedures should be done, and you can feel the resistance. This is understandable given the nature of the procedure, but some of them are willing. The problem is that further studies and screening cannot push on if people are reluctant to begin with.  If colorectal cancer were symptomatic, then it would be easy.  Unfortunately, we’re talking about screening, which means that sometimes, people display no symptoms at all.  But they are nonetheless at risk,” said Dy.

“In general, colonoscopy is not the only thing we do to detect colon cancer or precursor lesions, but it is probably the gold standard at the moment.  For example, we can do testing for stools for blood or fecal DNA. But if the tests shows a positive result, then one should be ready for colonoscopy to clear the colon of any possible serious disease” Dy explained.

More people should be involved and be knowledgeable about colorectal cancer.  Current academic literature shows that the disease is becoming more pronounced with each passing year, with the Philippines having the highest increase in mortality rate in the world.  But this is mostly largely due to our passiveness to general medical check-ups. This is not a good sign as the disease is preventable.

“Most people tend to be reactive—they will only act when they feel that there is a change in how they feel.  The problem is, early stages of colorectal cancer is symptom free, and as such, the individual must be proactive in taking steps to catch colorectal cancer before becoming full-blown and too difficult to cure.  More people should be aware about it, and more people should know how lifestyle affects it,” Dy said.

As more Filipinos incline towards a more Western lifestyle and diet, colorectal cancer has become a real concern, it as risky and damaging as other types of cancer.  It is preventable, if proper screening test are performed for at risk individuals.  The recent data and international health study are a wakeup call for Filipinos to take colon cancer as seriously as what other Western countries are currently doing, and to consciously adopt a healthy lifestyle of proper diet and exercise.

Goldilocks levels up on-the-spot cake creations at the 11th ICDC Competition


It  was perhaps the sweetest college competition of the year, and unlike the sweat and adrenaline infused battles of the UAAP, this one is all about the sweet creativity of the competing schools.
Goldilocks, the country’s number one bakeshop, continues its tradition of honing creativity as it once again transformed cake into canvas at the the 11th installment of its annual Intercollegiate Cake Decorating Challenge (lCDC).






Held last Feb 18 at the Trinoma Activity center, the ICDC is recognized as the only cake decorating competition in the country wherein students from across the nation compete head to head with their on-the-spot cake creations. Given the more challenging parameters of this year’s contest, teams will be given five hours to decorate three cakes with corresponding themes.




And after all those kilos of sugar and flour were baked and the icing in place, St. Anne College of Lucena once again emerged as the overall champion of the ICDC. And got to bring home a baking showcase worth a quarter of a million pesos for their school, and P20,000 worth of Goldilocks gift certificates for themselves



THE FESTIVAL THEME CATEGORY WINNER
Nearly one hundred entries were pre-judged to find the top ten qualifying teams for 2017 and after a comprehensive deliberation, the competing schools in the finals include the Technological University of the Philippines, Asian College Quezon City, Pangasinan State University, Lyceum University Laguna, St. Anne College of Lucena, Calayan Educational Foundation lnc., West Visayas State University, University of Cebu, Western institute of Technology-lloilo, and Gabriel Taborin College of Davao.

St. Anne College of Lucena successfully defended their title, emerging as back to back overall champions. St. Anne won both the Festival Theme and Travel Theme categories while the Western Institute of Iloilo had the best cake in the Wedding Category.

As seen in the past, this highly innovative competition not only requires the technical know-how of cake decorating. but also the creativity, originality, and the imagination needed to produce marvelous and eye-catching cake designs. The lCDC will be judged by some of the best and brightest icons in the baking and design industry to select the Winning team.


An exciting twist this year is the integration of an overarching theme, which is the fusion of the arts. This means that the competing teams will have to integrate the arts of fashion, sculpture, and painting into their cake creations.

“We set the theme of the ICDC to be fusion of the arts because we felt it was important to show that different types of art forms can come together, complement, and integrate with each other to form something unique and beautiful,” noted Goldilocks Marketing Director Cherry Caluya.




Although this may seem like a difficult challenge, the competitors will not be left to their own devices. Prior to the competition, the teams attended workshops hosted by experts in the fields of fashion design, sculpture, and visual arts.



“We made sure to equip our competing cake decorators with different perspectives involving different fields of creativity,” Caluya pointed out “We want to highlight the fact that the intricate and highly expressive art of cake decoration can also be a platform for one’s passion and individual expression, much like any other form of art,” she further emphasized.

The Philippines celebrates the 8th National Rare Disease Week on 22-28 February 2017



University of the Philippines National Institute for Health (NIH) partners with the Department of Health (DOH) launches 1st Philippine Rare Disease Symposium on February 22, 2017 at the Century Park Hotel Manila. The event was attended by DOH Sec. Paulyn Ubial, Dr Carmencita Padilla, Dr. Anthony Calibo and media.

Getting screened before you try to get pregnant can give you reassurance and can help you make an informed game plan for pregnancy. If it turns out that you are both carriers, you can be prepared for and bone up on what it means to have a baby with the genetic condition.




The Philippines celebrates the 8th National Rare Disease Week on 22-28 February 2017 which, together with the global commemoration of the Rare Disease Day on February . 28,2017, highlights the need to intensify the country’s efforts to provide adequate health care and government support to persons afflicted with a Rare Disease otherwise known as orphan disorder.



An orphan disease is defined as a condition that affects fewer than 200,000 people nationwide. This includes diseases as familiar as cystic fibrosis, Lou Gehrig's disease, and Tourette's syndrome, and as unfamiliar as Hamburger disease, Job syndrome, and acromegaly, or "gigantism." Some diseases have patient populations of fewer than a hundred. Collectively, however, they affect as many as 25 million Americans, according to the National Institutes of Health (NIH), and that makes the diseases--and finding treatments for them--a serious public health concern.



In the Philippines, a disease is considered rare if it affects 1 in every 20,000 individuals or less. Those afflicted with rare disease or orphan disorders suffer from social abandonment due to lack of existing network of support to aid them. The nature of rare disease is hardly known as well due to lack of information; and only few medical professionals in the country are aware of these disorders and how to diagnose and address these conditions.  Medical help is also elusive under the conditions of the country’s health priorities.



The country recently enacted   the Rare Diseases Act of 2016 (Republic Act 10747) which seeks to “improve the access of persons diagnosed to have a rare disease, or persons highly suspected of having a rare disease, to comprehensive medical care and to timely health information to help them cope with their condition.” 

The law mandates the Philippine Health Insurance Corporation (PhilHealth) to provide benefit package; medical assistance as provided in the Sin Tax Reform Act of 2012; fiscal incentives for donations intended for researches on rare diseases, maintenance of the Rare Disease Registry, or for purchase of orphan drugs or orphan products for use solely by persons with rare diseases as certified by FDA. Further, under RA 10747, patients with rare disease will also be considered as persons with disabilities and will be granted benefits and privileges such as priority programs and discounts as mandated by the Magna Carta for Disabled Persons.

Mr. Randy Martinez, recounts the challenges of going back and forth to the hospital as her daughter Pauline, seeks treatment for her Gaucher disease. Pauline who is now 11 years old, was noticed to have a small bump on her left torso when she was 6 months old and was later found out after years of  several tests to be Gaucher disease. “Sana magpatuloy pa ang paggamot kay Pauline at marami pang tumulong sa kanya para dire-diretso ang gamutan. Para makapag tapos din siya ng pag-aaral”, Pauline’s father said.
